The move from school to the adult world can present many challenges for young people. Best outcomes occur when the process is initiated early. A team approach where the student, family member, advocate, community, behavioral health team, vocational rehabilitation counselor and case manager can meet and plan together for long term goals is ideal. Such planning should be happening for youth with disabilities as a part of the IEP process.
Today we will meet a young graduate and ask him a few questions:
- What helped you to be most successful?
- What are you most looking forward to after high school?
- What worries you the most?
- Who will you contact if you are struggling?
